Born October 28, 1980, in Logan, Ohio.
Son of Brenda Jean (Myers) Heidlebaugh of  Logan, and the late Michael Richard Slack.

Mr. Slack had worked as a floor installer in building trades construction.

Funeral service will be held at 1:00 p.m., Wednesday, December 24, 2008, in the Roberts Funeral Home, Logan, with Rev. Mark E. Barrell, officiating.

Burial will be in New Fairview Memorial Gardens, Good-Hope Township, Hocking County, Ohio.

Friends may call Tuesday from 6-8 pm at the funeral home and Wednesday from 9:00 am until time of service at the funeral home.
