Jordan Renae Cook Chaffin, aged 30 of Logan, Ohio, passed away December 22, 2025.
Jordan,
Those who knew you knew right away you carried a fire inside you that couldn’t be dimmed. When you set your mind to something, there was no stopping you. You didn’t speak just to be heard, but when you did speak, people listened. Your words had weight, and your presence had power. You grew up strong, competitive, and determined, dominating the cheer mat and never backing down from a challenge. You weren’t afraid to work hard or get your hands dirty you actually thrived in it. You loved the outdoors, mud runs, and a life that didn’t require dressing things up. A T-shirt and a good pair of boots were always enough for you.
You were loyal in a way to your friends and family that’s rare, loving in a way that was fierce, and you had a heart that beat especially hard for animals and those who needed protection. But above everything else above all roles and titles you were a mother. Loving Zayden was your greatest purpose, your proudest role, and your deepest joy. Every part of you lives on through her.
You leave behind your beloved daughter, Zayden; your mother, Heather Cook; your father, Greg Forrest; your grandparents, Barney & Molly Cook and Natalie Simpson; your special aunt, Nan Flowers; your special uncle, Mike Flowers; and a very large, close, and loving family who will forever hold your memory tight.
You were proceeded in death by your great grandmother Phylis; your mom’s best friend, Aaron St. Clair, who helped play a role in raising you, whose name you passed on to your daughter; and Jason Tipton, who earned the title of Gumpy to Zayden.
A funeral service officiated by Pastor Micah McCoy will be held at the Roberts Funeral Home in Logan, Ohio at 11 AM on Saturday, January 3, 2026. Burial will follow in Oak Grove Cemetery in Logan, Ohio.
Calling hours will be held at Roberts Funeral Home on Saturday, January 3, 2026 from 9 AM until time of service.
Jordan, your love and light live on every single day through your daughter and through the many lives you touched. You will be missed beyond measure, loved beyond words, and remembered always.
